Lucknow: Uttar Pradesh's Chief Electoral Officer (CEO) Navdeep Rinwa on Tuesday announced that people applying for voter registration under the Special Intensive Revision (SIR) 2026 should submit a mandatory declaration form along with Form 6.

What did the UP CEO say?

The CEO said that the applications submitted through Form 6 for registration in the electoral roll will mapped as part of the SIR process.

Navdeep Rinwa said that applicants will be required to submit a declaration form providing details of themselves and either their parents or grandparents, along with the assembly constituency number, part number and serial number from the electoral roll published in 2003.

Rinwa said that "about 25.08 lakh Form 6 applications had been submitted till Tuesday (January 20). Before the publication of the draft electoral roll on January 6, as many as 16,18,574 applications were received. After the publication of the draft roll, 8,74,976 Form 6 applications were submitted till Tuesday, while political parties submitted 14,627 Form 6 applications for enrolment of new voters."

What details declaration form needs?

The voter must submit the declaration form with the complete details, including, their full name and current address with correct spelling, a recent photograph, and a valid mobile number. Additionally the details from the 2023 voter list of either their parents or grandparents with the assembly constituency number, part number and serial number.

The applicants who correctly map these information's will proceed through any issue but if there is any issue or data is missing than a notice from the Electoral Registration Officer (ERO) will issued.

In such cases, the applicant will have to appear before the Booth Level Officer with the  documents prescribed by the Election Commission of India (ECI) as proof of date of birth and/or place of birth.

Documents required to verify are:

  • Birth certificate issued by the competent authority
  • Aadhaar Card
  • PAN Card
  • Driving License
  • High School/Intermediate certificate mentioning the date of birth
  • Indian Passport

And in case any of the above required documents are not available, the Election Commission will conduct an on-site verification to confirm the applicant's eligibility.
